You need to fillet an editable_spline.
Create an editable spline, for exmpl. rectangle and convert it to editable spline.
Select all knots and click the fillet button, drag and drop to adjust the fillet threshold. Now apply a bevel modifier, adjust your height, start outline and outline. Maybe this can help you.

here is an exemple, left is chamfer at 0.5 tension ( I tried all values ) and right is the mesh I am looking for...

I can't quite visualise the problem but have you tried lowering the Tension ?
(you'll have to use Quad Chamfer to enable Tension)
